OLPH Elementary & Middle School Faith Formation

The Elementary & Middle School Faith Formation program at Our Lady of Perpetual Help Parish exists to support families in raising children who know, love, and follow Jesus Christ. We believe that parents are the primary catechists of their children, and our program is here to assist and accompany them in that sacred responsibility. Through engaging catechesis, prayer, Scripture, and preparation for the sacraments, we help children deepen their understanding of the Catholic faith. Regular participation at Sunday Mass is an essential part of faith formation, as the Eucharist is the source and summit of our Christian life. Together, as a parish family, we partner with parents to foster lifelong disciples who live out their faith with joy and conviction.

At Our Lady of Perpetual Help, we believe that parents are the primary formators of their children’s Catholic faith. Our parish program is here to support, encourage, and equip families as they help their children grow in faith, prayer, love for the Sacraments, and friendship with Jesus Christ.

Faith Formation is more than a class. It is a partnership between the parish and the home. Through age-appropriate lessons, sacramental preparation, community, and family support, we want each child to know that they are loved by God and called to live as a disciple of Christ.

Before You Begin

  • One form per child being registered.

  • Have parent/guardian contact information ready.

  • Have sacramental information ready, including Baptism, First Communion, and First Reconciliation, if applicable.

  • A Baptism certificate may be requested for children preparing for First Reconciliation or First Communion.

  • Emergency contact and medical information will be collected for your child’s safety.

  • Faith Formation is a partnership between the parish and the home.

  • Payment may be completed through this form.

Weekly Sunday Schedule

9:00 a.m. – Families are encouraged to attend Sunday Mass.
10:30 a.m. – Student Check-In (Parents sign students in.)
10:30–11:45 a.m. – Faith Formation Class
11:45 a.m. – Dismissal (Parents must sign students out.) 

Elementary & Middle School Faith Formation Classes are held on Sundays from 10:30 AM–11:45 AM.

Upon arrival, parents or guardians must check their child in before class. Students will then proceed directly to their assigned classroom. At the conclusion of class, parents or guardians are required to check their child out from their classroom. For the safety of all children, students may only be dismissed to an authorized adult.

Parent Pickup Requirement:

Parents and guardians are required to pick up their children promptly at 11:45 AM from their designated classroom. Children may not be escorted to the church by catechists or parish staff.
